To ensure that you and your family receive the best care possible, we assign a case-management supervisor and patient-care coordinator to support your family. Your case-management supervisor will manage your care, provide you with progress updates, and establish ongoing communication with your doctors, therapists and other members of the health care team. Your care-management team will also submit all of the necessary paperwork to insurers and Medicaid. They are always available to you.
Your patient-care coordinator is responsible to ensure that you have a consistent, completed schedule, and stays in touch with you on a regular basis regarding care. Supervisory, clinical and schedule-coordination support are available to you 24 hours a day.
